Welcome to the Dominican University Patient Portal

Use your Dominican University Black Board and Portal user name and password to login (Usually FirstName.LastName1, do not use @dc.edu or @duny.edu on the end).  

Once logged in you will have access to online services and forms such as:

• Input and obtain immunization records*
• Upload immunization records and other health documents
• Complete health history forms, review and sign HIPAA forms
• Review Vaccine Information Sheets (VIS) and sign vaccine consent forms
• Telemedicine information sheet to complete before each telemedicine visit
• Telemedicine consent once a year
• Report positive COVID-19 case
• Input insurance information (required for athletes only)
• Complete and upload athletics documents
• View patient specific education materials based on your visits to the Health Center

* After you input your immunization dates, please also remember you are required to provide a copy of your official Immunization Record.  

Students under the age of 18 need parental consent to be seen at the Student Health Center.  Please download the Parental Consent for Student Under age 18 form found under the Forms section. The PDF is under the downloadable forms section.  It must be printed, signed by your parent or guardian and uploaded back into the Medicat patient portal.

We are committed to protecting your personal information. Data that you provide cannot be viewed by anyone else on the Web and is securely maintained by industry standard SSL (secure socket layer) encryption and decryption technology when needed. We do not share your information with anyone else.
